What this relationship means: Acts after mast-cell mediators are released rather than stabilising the mast cell itself. This includes treatments that block mediator receptors, degrade a mediator, or oppose the physiology of an acute reaction. It describes where the intervention acts, not its clinical importance. A 2025 practical-management review places this at step 1 of the sequence it describes — described as the initial treatment, with second-generation agents prioritised over first. Mechanism of action
Cetirizine occupies the histamine H1 receptor, preventing histamine already released from mast cells from producing its downstream effects — vasodilation, increased vascular permeability, pruritus, and sensory nerve stimulation. It acts on the mediator’s target rather than on the cell, so it does nothing to reduce degranulation itself.
Its randomised evidence comes from chronic spontaneous urticaria and allergic rhinitis rather than from MCAS. H1 blockade is the first step in the stepwise mediator-blockade approach described in the mast cell disease management literature.
