What this relationship means: Studied in people diagnosed with mast cell activation syndrome. Current evidence consists of case reports, uncontrolled series, or retrospective review rather than randomised comparisons — so it establishes that people have taken it, not that it worked.
Mechanism of action
“GLP-1-based medicines” is a class label rather than one interchangeable drug. Semaglutide is a GLP-1-receptor agonist, while tirzepatide activates both GIP and GLP-1 receptors. Formulations in this broad group have metabolic indications and distinct labels; none has a US indication for MCAS.
A 2025 retrospective publication assembled 47 people diagnosed with MCAS from the authors’ patient panels after exposure to several GLP-1-based medicines. The paper states that the cases came from those panels and presents a representative case from each author. That provenance — cases drawn from specialist practices rather than an unselected population — is what the documented-specialist-use tag records. The authors are named in the citation below. The retrospective, selected, uncontrolled design is unchanged by any of it.
The mechanistic bridge is unsettled. GLP-1 signalling can alter metabolism, gastric emptying, appetite, inflammatory networks, and multiple immune-cell pathways. A separate two-case report in chronic spontaneous urticaria notes that mast-cell GLP-1-receptor expression is established in rodents but conflicting in humans, and discusses weight, diet, gut, inflammasome, and other indirect explanations. This page therefore does not label the class a mast-cell stabiliser.
The approval rows are formulation-specific examples, not a class-wide approval statement. The current semaglutide and tirzepatide labels include major contraindications and warnings, and their use for MCAS remains off-label.
