What this relationship means: Studied in patients with a different mast cell disease, usually systemic mastocytosis. Those disorders have diagnostic criteria and disease biology that differ from MCAS, so a result there does not establish an MCAS outcome. A 2025 practical-management review places this at step 1 of the sequence it describes — described as the initial treatment, with second-generation agents prioritised over first. That is what the review reports, not a recommendation from this site.
Mechanism of action
Hydroxyzine is a first-generation H1 receptor antagonist. It occupies the receptor that histamine would activate after mast-cell release, so its placement here is downstream of the mast cell rather than as a mast-cell stabiliser.
The current US label lists histamine-mediated pruritus, including chronic urticaria, among its uses. The mast-cell-disease trial compared hydroxyzine, ketotifen, and placebo in eight children with mastocytosis using a crossover design. Its age, size, and population make it a narrow evidence source rather than an MCAS trial.
Unlike less-sedating second-generation H1 antihistamines, hydroxyzine readily produces central nervous system effects. The current label’s drowsiness, CNS-depressant interaction, and cardiac-rhythm warnings are practical distinctions that remain visible alongside the study context rather than changing it.
