What this relationship means: Studied in people diagnosed with mast cell activation syndrome. Current evidence consists of case reports, uncontrolled series, or retrospective review rather than randomised comparisons — so it establishes that people have taken it, not that it worked. A 2025 practical-management review places this at step 8 of the sequence it describes — described for severe recurrent reactions despite the preceding treatments. That is what the review reports, not a recommendation from this site.
Mechanism of action
Omalizumab binds circulating IgE at the site that would otherwise engage the high-affinity receptor FcεRI, lowering free IgE. Sustained reduction in free IgE leads to downregulation of FcεRI expression on mast cells and basophils, raising the threshold for IgE-mediated degranulation.
Its US label covers allergic asthma, chronic spontaneous urticaria, chronic rhinosinusitis with nasal polyps, and IgE-mediated food allergy — not mast cell activation syndrome. Use in MCAS is off-label.
The two facts on this page are worth holding apart. Omalizumab has randomised, placebo-controlled trials behind it, and those trials are in chronic spontaneous urticaria and in IgE-mediated food allergy. In MCAS itself the published record is uncontrolled. A reader who takes “there are randomised trials” and “this is used for MCAS” and puts them together gets a conclusion neither fact supports.
